Home Health 101

Thank you for taking a moment to learn about what it means to be homebound and how home health care can provide treatment in the comfort of familiar surroundings.

Most important is to realize home health care is a temporary situation while you are being restored to your strongest self and that you must be under the care of a doctor to receive services.

Ohana Home Health provides temporary support through skilled services. The temporary part is very important. We do not provide private nursing or long-term care. Skilled services include physical, speech and occupational therapies; nursing, behavioral health and home safety assessments.

Remember, you must have a physician’s referral to utilize our services and we do not cover outpatient services. An “outpatient” is a patient who receives medical treatment without being admitted to a hospital.

Once we receive your physician’s referral, we send in one of our healthcare team to perform an assessment, which is a big word for a simple head-to-toe evaluation used to develop a care plan based on your needs. Once we have your plan of care we schedule therapists to visit your home.

According to the Center for Medicare Services, homebound means:

  • Leaving your home isn’t recommended because of your condition.
  • Your condition keeps you from leaving home without help (such as using a wheelchair or walker, needing special transportation, or getting help from another person).
  • Leaving home takes a considerable and taxing effort.

What Ohana Home Health is: We are a strong rehabilitation team comprised of licensed therapists supporting the homebound patient using skilled services.

What we are not: We are not private duty nursing for continuous care. For this reason we can not stress enough the importance of family involvement and training while we are in the home.
